HIPAA Notice of Privacy Practices

Effective date: August 24, 2026

Your information. Your rights. Our responsibilities.

This notice describes how medical information about you may be used and disclosed, how you can obtain access to this information, and our responsibilities concerning your information. Please review it carefully.

This notice applies to Coastal Medical and Vitality and the healthcare professionals who provide services through the practice, as applicable.

Your rights

You generally have the right to:

Obtain a copy of your medical record

You may request an electronic or paper copy of your medical record and other health information we maintain about you. We will provide a copy or summary within the time required by law. A reasonable, cost-based fee may apply where permitted.

Ask us to correct your record

You may ask us to correct health information you believe is incorrect or incomplete. We may deny the request in some circumstances, but we will explain the reason in writing as required by law.

Request confidential communications

You may ask us to contact you in a particular way or at a particular location. We will accommodate reasonable requests.

Ask us to limit certain uses or disclosures

You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations. We are not always required to agree. If you pay in full out of pocket for a service or item, you may ask us not to disclose information about that service or item to a health plan for payment or healthcare operations, unless disclosure is required by law.

Obtain a list of certain disclosures

You may request an accounting of certain disclosures of your health information made during the period permitted by law. The accounting does not include every disclosure, such as many disclosures for treatment, payment, or healthcare operations.

Obtain a copy of this notice

You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.

Choose someone to act for you

If you have given someone medical power of attorney, or if someone is your legal guardian or otherwise legally authorized, that person may exercise your rights. We will verify the person’s authority before acting.

File a complaint

You may complain if you believe your privacy rights have been violated. Contact us using the information below. You may also submit a complaint to the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Your choices

For certain health information, you may tell us your preferences concerning disclosures to family, friends, or others involved in your care; disaster-relief situations; fundraising; or other situations allowed by law. If you cannot communicate your preference, we may act in your best interest where legally permitted.

We will obtain your written authorization for uses and disclosures that require authorization, including most uses of psychotherapy notes, most marketing uses involving payment, and sales of protected health information. You may revoke an authorization in writing, except to the extent we have already relied on it.

How we may use and disclose health information

We may use or disclose your health information as permitted or required by law, including to:

  • Treat you and coordinate care with other healthcare professionals;
  • Bill for services and obtain payment;
  • Operate our practice, improve care, train staff, and conduct quality activities;
  • Contact you about appointments, treatment alternatives, or health-related services;
  • Assist with public-health and safety activities;
  • Report suspected abuse, neglect, or domestic violence when permitted or required;
  • Comply with health-oversight activities, audits, investigations, or licensing requirements;
  • Respond to lawsuits, court orders, subpoenas, or other lawful process;
  • Work with coroners, medical examiners, funeral directors, and organ-procurement organizations;
  • Address workers’ compensation, law-enforcement, correctional, national-security, or protective-service purposes where authorized; and
  • Comply with other legal requirements.

Where stricter federal or state protections apply—including protections for certain substance-use-disorder, mental-health, genetic, reproductive-health, or other specially protected records—we will follow those requirements.

Our responsibilities

We are required by law to:

  • Maintain the privacy and security of protected health information;
  • Provide this notice describing our legal duties and privacy practices;
  • Follow the notice currently in effect;
  • Notify affected individuals following a breach of unsecured protected health information when required; and
  • Refrain from retaliating against anyone who exercises a privacy right or files a complaint.

We may revise this notice and make the revised terms effective for all information we maintain. The current notice will be available at our office and on our Website.
